Class 1 – Who we are and what we do

Practice details, information about our Practice team and our opening times are all published on our website. This information can be accessed via the About Us page. 

Class 2 – What we spend and how we spend it

The Practice receives money from NHS Wales according to its contract for national General Medical Services in exchange for services provided for patients.

This information can be obtained on written request to the Practice by writing to Caergwrle Medical Practice, 39-41 High Street, Caergwrle, Wrexham, LL12 9LG, or by hand delivering a written request to our Management Team. Alternatively, please contact our Management Team on 01978 760413 if you have any queries. 

There may be circumstances where material cannot be released because it is confidential or commercial information or the appropriate officer designated for these purposes, under the Act, has taken the view that it may be prejudicial to the conduct of the Practice’s affairs.

If this is the case, we will respond to your FOI request with a formal letter acknowledging the reasons why we are unable to give you this information.

Class 3 – What our priorities are and how we are doing

  • Working closely with our colleagues in the South Flintshire Cluster to develop and improve services for patients in the local area. 
  • Working towards achieving required standards such as Quality Assurance Framework (QAIF), Access Standards, Medicines Management, Patient Safety, IG toolkit and Clinical Governance Practice Self-Assessment Tool (CGPSAT). 
  • Working with our neighbouring practices and the Health Board to continuously improve the services that are offered. 
  • The practice have not had a recent inspection from Health Inspectorate Wales (HIW). When we do have an inspection, the report will be published here. 
  • We do not currently have a Patient Participation Group (PPG) 

Class 4 – How we make decisions

Management policies are formulated at regular meetings. Decisions affecting the provision of NHS services are recorded. These are available on written request (any information which is commercially sensitive or falls under the Data Protection Act is excluded).

Class 5 – Our policies and procedures

Our policies and procedures are available on written request to the Office Manager. The practice policies include but are not limited to:

  • Call Recording Policy
  • CCTV Usage and Monitoring Policy
  • Complaints Policies and Procedures
  • Confidentiality Policy
  • Health and Safety Policy
  • Internal policies and procedures for staff relating to delivery of services
  • Policies and procedures for handling requests for information including Subject Access Requests (more information can be found HERE)
  • Policies and procedures regarding employment of staff 
  • Zero Tolerance Policy
